Request Certificate of Insurance:

In order for a certificate of insurance to be issued we need to following information:

1)Name of requesting party/entity

2)Address of requesting party/entity

3)Email address for certificate delivery

Special Note: A request that includes special/revised insurance or certificate wording and/or actual copy of policy endorsement showing requestor/s information to be issued is subject to underwriter approval and an additional charge of $250.00

THE GAME TRUCK QUOTING INFORMATION QUESTIONNAIRE:

The following information is required to obtain an insurance offer/quote:

>Business entity name and corporate status.

>Contact Phone number/s and Email address

>FEIN (Tax ID number)

>Business address.

>Mailing address if not the business address.

>Address for any alternate storage location of property or trailers.

Business Location: We require a physical location other than a PO Box. It may be your
home office.

>How many square feet is it?

>Is location constructed of Frame / Masonry / Steel?

>Does this location have a Central Station Alarm?

>Does this location have a Fire Sprinkler System?

>How much in Business Property/Contents do you have in the office?

>Does your lease specify responsibility for insuring the Tenant Improvements or HVAC?

>What is the name and address of your landlord?

General Liability:

>What are your estimated Gross Receipts on an annual basis?

>What is your estimated annual employee payroll?

Commercial Auto:

>Year, Make, Model and Vehicle ID Number (VIN) for each vehicle to be insured.

>What is the cost new of this vehicle?

>Cost of customization? Including any graphics/wrap, hitch, suspension?

>Year, Make, Model, VIN and length for each trailer to be insured.

>What is the cost new of the trailer including any graphics/wrap?

>What is the value of Electronics and Equipment installed in or on the trailer?

>Full Name of operators

>Date of Birth (DRIVERS UNDER 25 ARE AN ISSUE)
